Where is my exemption application? Most organizations seeking recognition of exemption from federal income tax must use specific application forms prescribed by the IRS. Forms currently required by the IRS are Form 1023, Application for Recognition of Exemption Under Section 501(c)(3) of the Internal Revenue Code, Form 1023-EZ, Streamlined Application for Recognition of Exemption Under Section 501(c)(3) of the Internal Revenue Code, Form 1024, Application for Recognition of Exemption Under Section 501(a) or Section 521 of the Internal Revenue Code and Form 1024-A, Application for Recognition of Exemption Under Section 501(c)(4) of the Internal Revenue Code, to request recognition of tax-exempt status. The law requires the payment of a user fee for determination letter requests such as your application for recognition of tax-exempt status. Your payment must accompany your request.
