Before you begin your CE Provider application, be sure you have the following available: Information about your organization (name, address, EIN) Contact information for a primary point of contact for your organization Supplemental materials (view this chart for the requirements specific to your organization type) For each CE program offered: Program title Description (view samples PDF) Target audience (e.g., EA, ERPA) Category (e.g. ethics, federal tax/tax related matters, etc.) Delivery method (e.g., in-person, self-study, online) Number of CE hours received Credit card, Paypal or bank account payment information to process your non-refundable $650 application fee
