Pictured is a completed Form 8269, Notice of Dishonored Check for More than One Tax Form. This form is sent to the taxpayer with the dishonored check and Form 2287/Form 2287 (SP) when one check is sent as remittance for more than one tax form. The form contains the amount of the returned check, penalty, and total due with Form 2287/Form 2287 (SP). The notice says Enclosed is your check which was not accepted by your bank. Also enclosed are separate Form 2287/Form 2287 (SP), Notice of Check Not Accepted By Bank, for each tax or taxable period covered by the check. The Form 2287 /Form 2287 (SP) for the largest amount also includes the penalty that applies to the check. Please return part 2 of each Form 2287/Form 2287 (SP) with your repayment so we can promptly credit your account for each tax or taxable period. This type check is called a split remittance.
