Preparation of Form 813-A from Forms 813 (Manual Deposit)Illustration of prepared Form 813-A from a Form 813 for manual deposits. A Form 813 is shown which will list the amount of each check that is deposited under an assigned Document Locator Number (DLN). The Form 813 will list the total of each amount at the bottom of the form. Also shown is the Form 813-A which will list each total of all Forms 813 that are associated with a deposit ticket by Tax Class. The Tax Class is the third position of the DLN and identifies what type of payment is being made. This illustration shows the total of the Form 813 listed in Tax Class 1 of Form 813-A. which is associated with taxes Withheld and F.I.C.A. taxes. Form 813 totals listed under the appropriate tax class are then totaled by tax class. The totals of each tax class are then listed in the Summary for Deposit Ticket section of the Form 813-A by tax class. Each tax class in the Summary for Deposit Ticket section is then totaled and should balance to the total of the associated deposit ticket. Copies of the Form 813s and the associated Form 813-A are forwarded to the Revenue Accounting Control Section (RACS) for journalizing into the RACS System. The RACS System will account for all monies deposited with the IRS and should balance with monies deposited with the depository banks which are reported to Treasury.
